In the June 2018 issue of the Safety Institute of Australia’s publication ‘OHS Professional’ BMA’s Tim Edwards and Colin Toogood describe a litany of occupational health and safety transgressions from the earliest days of the Union Carbide plant in Bhopal; and how a complex series of interacting events including design defects, repeated failures, reduced staff levels, inadequate training and missed warnings all contributed to the chemical holocaust of that night in December ’84.
